Export form

Show the live score panel to respondents in the exported form. Off by default — for clinical screeners, surfacing a real-time severity band to the client is usually not appropriate.

Contact types for this form

Tag this form with the contact / user types it applies to. Pick as many as you need — they show on the form in Forms and can be filtered there.

Contact types come from the EHR (Settings → Contact Types) and are shared across the platform. A type added here is added in the EHR too (EHR admins and managers). A type removed in the EHR is removed here too; records that already use it keep it.

Help & Guided Walkthroughs

Each feature has a short step-by-step walkthrough. New users see it automatically the first time they open that feature; turn any of them off here, or press Start to run one now.

Insert a standard screening form — questions, point values, severity bands, and risk alerts come prewired. Two groups are available below: scored screening forms, and unscored intake, assessment, and consent templates the client fills out. Everything is fully editable after inserting.
